Referral Process

Any young person undergoing cancer care in Victoria is eligible to access the services of the onTrac@PeterMac program. Referrals may come from a number of sources:

  • Medical referral (GP, Specialist)
  • Nursing staff (from PMCC or any other medical facility)
  • Self-referral
  • Allied Health (SW, OT, Physio, etc)
  • Referral by family member
  • Referral by any other interested party (eg. school, church, youth worker, etc)

Referral forms may be downloaded here. Referrals are also accepted via telephone, fax, or email. A completed referral is required prior to action being initiated by the onTrac@PeterMac team.

All sections on the form should be completed to enable an accurate assessment of the best way to provide support to the young person being referred. Inclusion of a fax number will ensure an acknowledgement of the referral can be made.

Epidemiology data collection

Epidemiological data regarding various aspects of the onTrac@PeterMac program are able to be extracted from the onTrac@PeterMac database, as necessary. Statistics providing various details of the onTrac@PeterMac program are calculated on a monthly basis, and ad hoc as required. Details such as the number of referrals, reasons for referral, sources of referral, etc. are reported on in graphic or tabular form, and may be included in presentations to users, or potential users, of the program.
